You will be asked to fill out health and medical information when you arrive at the clinic. You can print out and complete the following three forms prior to your visit if you prefer.
Before your visit, please bring your questions. You can also review plastic surgery procedure information and before and after photos on our website.
During your visit, you will receive a folder containing information about your procedure along with our price list. Dr Camp will ask you what your objectives are, perform a physical exam, and make recommendations. At the end of your visit you will receive a price estimate. After your visit, you will receive a letter from Dr Camp regarding your conversation.
